Be a partner
with your health care team and know what prescription and nonprescription
medications, herbals, and dietary supplements you are taking.
Be safe by
maintaining a current medication record and make it available to
your family.
Situations
when you need to have a list of your current medication include:
seeking medical care from your current physician or new physicians
seeking medical care at a hospital or clinic, especially if you
are out of town.
Be sure to show
your medication record to a nurse or physician during every hospital
preadmission or admission interview. This will help improve communication
with the health care team and help lower the risk of medication
errors.
Be aware that
your health care team needs to know about any medicines, vitamins,
herbals, teas, and dietary supplements that you take. These include:
- Prescription
medicines
- Medicines
that you can buy without a prescription, such as antacids, aspirin
and other pain relief medicine, cough syrup, laxatives, and nasal
sprays.
- Vitamins
and dietary supplements, such as black cohosh, glucosamine, ginkgo,
ginseng, and St. John's wort.
